The Commonwealth Coordinated Care Plus Waiver (CCC Plus Waiver) is a Home and Community-Based Services (HCBS) waiver offering supports and services to the following groups of individuals:

1.     Older adults;
2.     Individuals who have a physical disability; and/or
3.     Individuals who are chronically ill or severely impaired, having experienced loss of a vital body function, and who require substantial and ongoing skilled nursing care.

People who fall into any of the categories listed above must be able to show they need immediate waiver services in order to stay in the community. 

Once you are approved for the CCC Plus Waiver, you must complete a Medicaid application. The application should include only the information for the individual requesting Medicaid; do not include household information. If you are completing the application for a family member, be sure to answer all questions on their behalf, not your own.

You have the right to appeal any Medicaid decision or action that you disagree with. Appeals must be submitted within 30 days of the date of the decision or action. Click here to view Medicaid appeal information.
