Effective 7/1/2017 – The EDCD Waiver and the Technology Assisted Waiver combined to become the Commonwealth Coordinated Care (CCC Plus) Waiver.
In general, to request screening for this waiver, you contact the Department of Social Services in your county/city or the Department of Health in your county/city.
