CCC Plus Waiver Screening Request

The Commonwealth Coordinated Care Plus Waiver (CCC Plus Waiver) is a Home and Community-Based Services (HCBS) waiver offering supports and services to the following groups of individuals:

1.     Older adults;
2.     Individuals who have a physical disability; and/or
3.     Individuals who are chronically ill or severely impaired, having experienced loss of a vital body function, and who require substantial and ongoing skilled nursing care. To learn more about the services and application steps visit here.
